Community Health Initiatives Tool

Hospitals and health systems across New York state engage in many collaborative initiatives to improve the health and well-being of their communities. This tool highlights programs HANYS members have implemented that:

  • identify and address the health needs of the community;
  • use a collaborative approach, working with partners within the community;
  • are preventive, not part of acute care and usually take place outside of a clinic or hospital; and
  • are related to the New York State Prevention Agenda that aims to improve the health and well-being of all New Yorkers and promote health equity.

HANYS members and the public are encouraged to review these initiatives as a means of inspiring new ideas and to drive replication of these initiatives and their impacts across the state.

The information provided in this tool is sourced from nominations for HANYS' Community Health Improvement Award submitted between 2021 and 2024. Initiatives may be continuing as originally described, may have evolved or may be no longer active, and contact information may have changed.
